Iran’s Supreme Leader, Mojtaba Khamenei, has been unable to address the public since assuming power due to severe injuries sustained in an Israeli airstrike on February 28. According to a *New York Times* report, the 56-year-old leader suffered extensive burns to his face and lips, making speech difficult and necessitating future plastic surgery. Additionally, officials reveal that Khamenei is currently awaiting a prosthetic limb after undergoing multiple surgeries on one of his legs and his hand.

The Supreme Leader is currently operating from a secret hideout, with communication restricted to a “human chain” of couriers carrying handwritten, sealed envelopes to prevent Israeli intelligence from tracing his location. This isolation has reportedly shifted significant decision-making power to the Islamic Revolutionary Guard Corps (IRGC) generals. While President Masoud Pezeshkian is reportedly involved in his care, senior government officials generally avoid visiting the hideout for fear of being tracked and killed.

The Trump administration has pointed to these leadership fractures as a primary obstacle to peace negotiations. White House officials described a “battle between pragmatists and hardliners” in Tehran, leading President Trump to extend an indefinite ceasefire to allow the fractured regime to formulate a unified response. In the interim, the IRGC has reportedly tapped Parliament Speaker Mohammad Bagher Ghalibaf to lead high-level back-channel talks with Vice President JD Vance.
